What is Breast Augmentation Surgery?
Breast augmentation surgery is performed for women who want fuller and larger breasts, achieving a size and shape proportional to their body for a more aesthetic appearance. While commonly referred to as “breast enlargement,” the medically accurate term is breast augmentation. This procedure must be individually planned and, when performed by an experienced plastic surgeon using proper techniques and modern implants, it is highly safe with a relatively short recovery period.
Candidates for breast augmentation include women whose breasts did not reach their desired size after puberty, those who experienced sagging or volume loss after uncontrolled weight loss, and women whose initially full breasts have lost volume after breastfeeding. Each of these situations may require slightly different approaches.
For women whose breasts did not reach the desired size, breast augmentation can help achieve the preferred shape and volume. Except for certain special cases, these patients should be at least 18 years old and have largely completed breast development. Typically, these patients have minimal breast tissue, no sagging issues, and the desired volume and shape can usually be achieved with a simple procedure using submuscular silicone implants. In some younger patients with significant asymmetry between the breasts, additional procedures such as fat transfer may be required to achieve symmetry.
Women who initially had breasts with a size and shape they were happy with but experienced volume loss or tissue atrophy after breastfeeding are also good candidates for breast augmentation surgery. If the breast skin remains of normal size but the breast appears empty and saggy, this is called pseudoptosis, and it can be corrected simply by placing silicone implants. However, if there is excess skin and true sagging, the silicone implant should be combined with a breast lift (mastopexy) to achieve proper shaping and lifting.
Patients who have lost a significant amount of weight due to bariatric surgery (such as gastric sleeve),dieting, or other reasons may also experience breast deflation and sagging. These cases usually involve noticeable excess skin, requiring a combination of silicone implants and a lifting procedure. If sagging is also present in the upper torso, back, or arms, additional surgeries may be needed to achieve optimal results.
Breast augmentation surgery is not only a physical transformation but also an emotional and psychological renewal. This process helps women feel more confident, satisfied, and happier with their bodies.
Who Can Have Breast Augmentation Surgery?
Breast augmentation surgery can be performed on anyone whose breast development is largely complete but who is not satisfied with the appearance of their breasts and has no health issues preventing surgery.
Breast Augmentation Surgery Methods
Breast augmentation is performed using silicone breast implants. Various incision types and approaches are used to place the implant behind the breast tissue. Typically, a 3–4 cm incision is hidden in the breast crease (inframammary fold),allowing access behind the breast. Depending on the technique, the implant can be placed under the chest muscle, under the fascia above the muscle, or above the muscle (beneath the breast tissue). Modern techniques do not enter the natural breast tissue, ensuring no damage occurs to the breast itself.
How Is Breast Augmentation Surgery Performed?
Breast augmentation is usually done under general anesthesia. To minimize visible scarring, incisions are made either around the areola, in the armpit, or along the inframammary fold, depending on the implant size, patient preference, breast anatomy, and surgeon recommendation.
Since the implant placement avoids damage to breast tissue and milk ducts, women can safely breastfeed after surgery. After the incision, the silicone implant is positioned either beneath the chest muscle or between the muscle and breast tissue. The incision is then closed with aesthetic sutures.
In some cases, fat grafting can also be used for breast enlargement. This involves harvesting fat from the patient’s own body, processing it, and injecting it into the breast area. The surgeon determines whether this method is suitable. Small amounts of fat transfer may also be used to fine-tune or camouflage an implant. However, fully augmenting the breast using only fat grafting is generally not suitable for most patients.

Choosing Silicone Implants for Breast Augmentation
There are various manufacturers that produce silicone implants for breast augmentation surgery. Using a high-quality, reputable brand is crucial for ensuring long-term results and minimizing the risk of complications. The surgical technique and the surgeon’s experience also significantly impact the longevity of results and the likelihood of long-term issues.
All manufacturers offer a wide range of shapes and sizes. The main features of silicone breast implants to consider include shape, surface texture, filling type, and size.
- Implant Shapes: Implants are generally categorized as either teardrop (anatomical) or round. Both types may have variations in height or projection.
- Surface Texture: Implants are either textured (rough) or smooth. Textured implants can be further categorized based on the characteristics of their surface.
- Filling Material: Implants may be filled with either saline solution or silicone gel.
Saline-filled implants are generally not ideal for aesthetic purposes because they do not provide a natural breast feel and can present other issues.
Implants intended for cosmetic use are filled with silicone gel. Modern silicone gels are cohesive, meaning they are sticky and do not leak. They have a consistency similar to gummy candy. In the U.S., the popular “gummy bear” implants are a type of these new cohesive gel implants.
Breast Augmentation with Fat Grafting
We frequently use fat grafting in breast augmentation to make the results achieved with silicone implants appear more natural. However, enlarging the breasts solely with fat injections—without any silicone implants—is not very practical with current techniques. To reach the breast size achievable with silicone implants, fat transfers would typically need to be repeated 3–4 times.
Because this approach is impractical for the patient and the results for large volumes are difficult to predict, I generally do not recommend breast enlargement using only fat, except for minor corrections or small volume increases. I find fat injections very useful for minor adjustments, smoothing transitions, or making the overall result look more natural.
Preparation Before Breast Augmentation Surgery
For women without a special risk for breast cancer and with normal examination findings, breast imaging is not mandatory before aesthetic surgery if under 40 years old. Women aged 40 and above should have a recent mammogram within the last year. If unavailable, a mammogram and, if necessary, a breast ultrasound should be performed.
We recommend stopping smoking and all nicotine products 3–4 weeks before any aesthetic surgery. Smoking impairs wound healing, reduces blood circulation in tissues, and delays recovery. Alcohol should also be avoided 3–4 days before surgery.
If you are taking blood-thinning medications such as aspirin or warfarin (Coumadin) for medical reasons, adjustments, substitutions, or temporary discontinuation may be needed based on your individual condition. Over-the-counter aspirin use for minor issues like headaches should also be avoided 2–3 weeks prior to surgery.
Herbal teas, vitamins, medications, and supplements with unknown ingredients may also thin the blood and should be discontinued 2–3 weeks before surgery.
Post-Operative Care After Breast Augmentation
It is important to wear the medical bra provided after breast augmentation continuously. This helps prevent implant movement, reduces swelling, and ensures proper breast shaping.
During recovery, avoid lying on your breasts, refrain from strenuous movements that strain the breasts, and avoid tasks that heavily involve the arms.

Scars typically measure 3–4 cm, though about 3% of patients may have more noticeable scars due to genetic factors. Scar-reducing creams can help prevent and treat these.
Do silicone implants have a lifespan?
Most manufacturers provide a lifetime guarantee. However, the American Society of Plastic Surgeons recommends replacing implants every 15–20 years. If no additional problems arise, replacement surgery is usually simple, painless, and performed within the existing implant pocket.
At what age can breast augmentation be performed?
We generally prefer surgery after 18 years of age. There is no upper age limit for healthy women.
Does breast augmentation cause cancer?
Breast implants do not cause breast cancer. Rarely, a type of lymphoma associated with foreign body reactions may occur, with an estimated risk of approximately 1 in 35,000.
